Peer-run self-help program that gives advocacy, information, and hands-on assistance to mental health recipients as they move from the hospital to the community. Includes self-empowerment and counseling, as well as assistance with obtainging documents, identification cards, birth certificates, social security cards, legal and immigration referrals, information and resources on SSI, SSD, Medicaid, Madicare, food stamps, and reduced fare Metro Cards. Housing specialists are also on site to provide assistance and referrals.
